Who can get a certified copy of a Death Certificate?

In order to obtain a certified copy of a Death Certificate, you must be a parent, legal living spouse, adult child, adult sibling, next of kin, legal guardian or legal representative.  Valid ID and proper documentation must be presented in order to obtain a Death Certificate.

Mail-In Requests

To mail in a request for a Death Certificate, you will need to send a photocopy of your valid Driver's License, a completed request form, a money order payable to Henry County Probate Court and a self-addressed stamped envelope to the address below.

The fee for the first certified copy is $25.00  Additional copies ordered for the same person at the same time are $5.00 each.
