Completed Passport Application Form

  • Do not sign until instructed to do so.
  • Must be filled out in black ink only.

Evidence of U.S. Citizenship

Must submit 1 of the following:

  • Previously issued, undamaged, and fully valid U.S. Passport (adults only)
  • Certified U.S. Birth Certificate - must meet all of the following requirements:
    • Issued by the city, county or state of birth of applicant.
    • Lists applicant's full name, date and place of birth.
    • Lists parent(s) full name(s).
    • Has date filed with registrar's office.
    • Has registrar's signature.
    • Has seal of issuing authority.
    • Photocopies and notarized copies are not accepted.
  • Consular Report of Birth Abroad or Certification of Birth
  • Naturalization Certification
  • Certificate of Citizenship

Photo Identification

Must submit 1 of the following:

  • U.S. Government ID, such as a valid State Issued Driver's License, Government ID card or an ID card obtained from the Division of Motor Vehicles.
    • If ID was issued in a State other than Georgia, a second photo ID will be required.
  • Current undamaged U.S. Passport

Parental Consent - minors only

  • Under age 16 - * Birth Certificate, Consular Report of Birth Abroad or Certification of Birth, Naturalization Certification or Certificate of Citizenship with parental information required for all minors.
    • Both parents/guardians must appear in person to give consent.
      • If one parent/guardian is unable to appear in person, then Form DS-3053: Statement of Consent from the non-applying parent/guardian must be submitted with a front and back photocopy of the parent/guardian's photo ID.
      • If the child only has one parent/guardian, evidence of sole authority to apply for the child must be submitted with the application in the form of:
        • A Court Order granting sole legal custody to the applying parent,
        • A Court Order specifically permitting the applying parent to apply for the child's Passport,
        • A Judicial Declaration of Incompetence of the non-applying parent, or
        • A Death Certificate of the non-applying parent.
           
  • Ages 16 - 17 * Birth Certificate, Consular Report of Birth Abroad or Certification of Birth, Naturalization Certification or Certificate of Citizenship with parental informatino required for all minors.
    • At least 1 parent/guardian must appear in person to give consent.
      • If the parent/guardian is unable to appear in person, then Form DS-3053: Statement of Consent from the non-applying parent/guardian must be submitted with a front and back photocopy of the parent/guardian's photo ID.

Passport Photo

We do not take Passport photos in our office.
 

Passport photos must meet the following requirements:

  • Must be in color
  • Must be 2" X 2" in size
  • Must be printed on photo-quality paper
  • Must be taken within the past 6 months, showing current appearance
  • Must be a full face, front view
  • Must be taken on a plain white or off-white background
  • Must not have glasses or headgear

Passport Fees

2 separate forms of payment are due at the time of application.  See fees for breakdown.
 

  • U.S. Department of State's Fee
    • We accept check or money order only.  We cannot accept cash or credit/debit card payments for the U.S Department of State's fees. 
  • Probate Court's Fee
    • We accept Cash, Money Orders, Local Henry County Checks (must be over number 500, no starter checks), MasterCard or Visa debit/credit cards (card processing fee will apply).
